The card may not be reproduced and must be accompanied by a valid prescription for 30 capsules (in 5, 10 or 15 mg strength) of MERIDIA (sibutramine HCl monohydrate) C-IV Capsules. This card is valid for 30 to 90 capsules according to the terms on the face of this card.
